But in healthcare, efficiency without safety is dangerous. An inversion approach asks: “How could this interface cause patient harm?” This question reveals entirely different design priorities.

In one project redesigning a hospital medication system, the inversion analysis uncovered critical failure modes: doctors might select the wrong patient from a dropdown list, dosage fields might accept dangerous values, or similar-looking medication names might cause confusion.     • External failure modes: What could the environment do to us?

    • Systemic failure modes: How could the interaction between internal and external factors create problems?
      Technology limitations meeting user expectations, team capacity meeting project scope
    • Temporal failure modes: How could timing create issues?
      Moving too fast and missing quality, moving too slow and missing market opportunity

Step 3: Create anti-goals and constraints

Transform failure modes into explicit boundaries:

    • If “running out of budget” is a failure mode, create the anti-goal: “Never exceed 80% of allocated budget without stakeholder approval”
    • If “building features users don’t want” is a failure mode, create the constraint: “No feature development without user validation”

Anti-goals aren’t just negative thinking—they’re design constraints that guide positive action within safe boundaries.
